DOCUMENTATION REQUIRED

1. Certified copy of a Birth Certificate from the city, county, state Registrar or from the Bureau of Vital Records. All birth certificates MUST have a raised or multicolored seal and a file date that must be within one year of birth. NO HOSPITAL BIRTH CERTIFICATES.

2. A previous US Passport, regardless of when it was issued; however, if the passport was issued within the last 15 years and the applicant was 16 years or older, they may apply by mail using the DS-82 form. If the applicant’s name has changed from the previous passport, they must submit a CERTIFIED COPY OF THE COURT ORDER or MARRIAGE CERTIFICATE with the DS-82 application.

3. NATURALIZATION certificate of CITIZENSHIP.

4. CHILDREN, birth through 15 years old, require BOTH PARENTS’ signatures.  If both parents are unable to appear, one parent can submit a STATEMENT OF CONSENT along with a clear copy of that parent’s driver’s license. Children, 16 through 17 years old, require just ONE PARENT'S signature.  ALL MINORS AGES 0-17 YEARS OLD MUST APPEAR WITH PARENT.

IDENTIFICATION REQUIRED

Valid pictured DRIVERS LICENSE.

PHOTOS REQUIRED

Bring TWO (2) identical passport pictures, 2” X 2” in size, with a PLAIN WHITE BACKGROUND ONLY, made within the last six months.

NOTATIONS

The section “IN CASE OF EMERGENCY, NOTIFY” MUST be filled out on both the DS-11 and the DS-82.

PHOTOGRAPHS THAT DO NOT WITHSTAND 225 DEGREES FAHRENHEIT ARE NOT ACCEPTED.
